Neuter / Spay Agreement

The following contractual agreement is between_ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ (breeder), hereafter referred to as "Seller," and _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ , hereafter referred to as "Purchaser."
